How Do You Know Audit Findings Were Actually Fixed?

"How do we know the issues found during the audit have actually been fixed?" a client asked us.

Two weeks ago, my answer was: you don't.

Today, my answer is: let me show you.

The missing link: evidence

At most companies, an audit goes like this: walkthrough, report, actions assigned. Then someone says "done". And that's where the chain breaks. Done based on what? Who checked it? Where's the proof if an authority inspection comes around?

Assigning an action doesn't mean the problem is gone. Closing it out needs proof.

How it works from now on

Every corrective action raised during an inspection is sent to its owner as a task. When they've finished, ticking a box isn't enough: to close it, they have to upload evidence, such as a photo, a document or a short description.

The evidence is then reviewed by the person who wrote the report or by the EHS team. They accept it, and only then is the task closed. If it isn't sufficient, they send it back.

The full cycle in one place

The system now covers the entire audit lifecycle:

Reporting – done during the walkthrough, on site. Action assignment – every finding gets an owner immediately. Evidence – the owner proves the fix. Close-out – the reviewer accepts it and the task is closed with a full record.

No spreadsheets, no files sent back and forth. One system that keeps a traceable record of what happened, who did it and who verified it. That's exactly what you'll need in an internal or regulatory inspection.

Who is it for?

EHS teams running safety, environmental, 5S or energy walkthroughs. Internal auditors recording audits of their own areas in the same system. EHS consultants who want to manage audits consistently across multiple clients.

The audit module is part of our work permit management software, so permits and inspections run on a single platform.

See it on your own process
